State of Illinois Health Requirements for the 2026-27 School Year
Students will NOT be able to attend school after October 15th, 2026 until ALL required information is received.
Requirements by Grade Level
Ninth Grade Students:
Physical Exam (Sports physical is NOT acceptable)
Complete immunization record signed by medical provider
Dental Exam on Illinois form due by May 15, 2027 (dated within 18 months of the May 15th deadline)
Twelfth Grade Students:
Proof of receiving Meningococcal vaccine.
October 15, 2026 is the deadline for all required health information.
