State of Illinois Health Requirements for the 2023-24 School Year

Students will NOT be able to attend school after October 16, 2023, until ALL required information is received.

Requirements by Grade Level

Preschool Students:

• Physical Exam

• Complete Immunization record signed by medical provider: (4) DtaP,           

(3) Polio, (4) Hib, (3) Hepatitis B, (1) MMR, (1) Varicella

(4) Pneumococcal

Kindergarten Students:

• Physical Exam

• Complete Immunization record signed by medical provider: (5) DtaP,

(4) Polio, (4) Hib, (3) Hepatitis B, (2) MMR, (2) Varicella,

(4) Pneumococcal

• Dental Exam on Illinois form due May 15, 2024 (dated within 18 months of

May 15th deadline)

• Eye Exam on Illinois form dated within one year of the first day of school

Second Grade Students:

• Dental Exam on Illinois form due May 15, 2024 (dated within 18 months of May 15th deadline)

Sixth Grade Students:

• Physical Exam (Sports physical is NOT acceptable)

• Complete Immunization record signed by a medical provider, including

(1) TdaP, (1) Meningococcal (these are 6th grade requirements)

• Dental Exam on Illinois form due May 15, 2024 (dated within 18 months of May 15th deadline)

Tuesday, October 16, 2023, is the deadline for all required health information.
